Job Description:
The position is full time. The Case Manager is responsible for linking maternity residents to a variety of opportunities/services. To meet their needs across all life domains, and coordinate the efforts of all external providers involved in resident’s life and/or care so that the therapist’s treatment plans are supported. Keeping the youth’s best interest as the priority, monitoring the efficacy of adjunct/outside services and continually striving to enhance the efficacy of all services provided. Also provide aftercare services to previously discharged residents, intake screenings, and structured parenting classes twice weekly. This position requires flexible work hours that include two weekday evenings.
Qualifications for positions:
The position requires strong written and verbal communication skills, an ability to work with a diverse client population, a desire to work as part of a team, a sense of humor, a working knowledge of community resources and a passion for assisting young mothers and their children. Minimum requirements include at least 2 years child welfare experience and a Bachelor’s or Master’s degree in social work, psychology, or counseling. Familiarity with the impact that serious emotional disturbance, trauma, abuse, chronic homelessness and family discord can have on youth and with the operationalization of the concept of resiliency preferred.
